Existing PCT Health Check Provision

We appreciate that many PCTs will be delivering their health checks via GP practices and/or community pharmacies. However, this may not be the only (or best) way to identify and reach those individuals at most risk.

 

Alternative Health Check Provision

Attempting to reach people who do not routinely visit their GP or pharmacy requires alternative methods of engagement. 

 

“Outreach” is one of those methods - delivering health services in the locations where health inequalities exist, outside of a surgery or pharmacy setting, outside of surgery times.

 

SureHealthChecks are part of the value proposition offered by Surelines in working with NHS and business partners. Our focus is in outreach projects – reaching those individuals who do not readily access NHS services - and motivating life enhancing changes in behaviour.

Our Focus

At Surelines our focus is on bridging the gap and reducing health inequalities - providing a great service to those groups that are not readily accessing current service provision. We believe that this requires an innovative and fresh approach to these at risk individuals.

 

Getting individuals to attend a health check is only the start of the process, we believe in having culturally sensitive life enhancing contact with the person. Individuals feel relaxed and comfortable to have meaningful, relevant conversations with sufficient consultation time to discuss and ensure understanding – and to agree relevant lifestyle changes. These are key elements in delivering a successful service that will have desired health outcomes.

 

SureHealthChecks offer a real opportunity to make significant inroads into reducing health inequalities, including socio-economic, ethnic and gender inequalities.

Partnerships

Surelines actively engages with stakeholders to deliver an effective service that is of value and benefit to the community. This includes working with NHS colleagues, councils, local businesses, community groups, the third sector and support groups. SureHealthChecks build on our extensive experience in providing local primary prevention screening services to businesses, as well as delivering primary care based chronic disease management programmes (eg CHD and asthma).

 

Our success has been based on broad clinical experience and a motivational approach to health education and lifestyle changes.

 

Also, working in a true partnership with businesses and/or GP practices we are supporting their needs, but ensuring the needs of the patient are paramount.

Our Objective

Within this service, Surelines’ sole objective is to develop and deliver Health Checks that fit your local need. The size and scope of the project will vary depending on the nature of your requirements. Our teams have the flexibility to deliver Health Checks in all manner of locations - at any time day or night.

Health Checks at Work for Council Staff

STAFF at Colchester Council were among the first employees to benefit form free work-place Health checks.

The ground-breaking initiative has been set up by NHS North East Essex and is part of the national NHS Health Check screening programme to detect individuals in the 40 to 74-year-old group who are at risk of developing cardiovascular disease.

The check is a standard assessment made up of various questions and measurements looking at age, gender, height, weight, body mass index, smoking, levels of physical activity, ethnic background, family history, blood pressure, cholesterol and glucose levels.

Colchester Council staff took advantage of the checks when nurses visited Rowan House to carry out checks for blood pressure, cholesterol and blood sugar levels.

Martin Hunt, deputy leader of the Colchester Borough Council, said: “The council is committed to ensuring the wellbeing of all its employees. We are grateful to the NHS and Surelines for providing these health checks to council staff, and helping to promote the importance of wellbeing in the workplace.”
